PASSPORT SERVICES
 
 
The US Embassy in Windhoek issues passports to American citizens applying for the first time, renewing an expired/expiring passport, or applying for a replacement for a lost or stolen passport.  Passport applications are normally processed in ten working days. In cases of genuine emergency, however, a passport can often be issued on the same day the application is approved. The US Embassy also can supply additional passport visa pages at no cost.
 
How do I apply for a US passport?
 
There are three types of passport application forms. The one you use will depend on the type of application being made. The form (DS-82) is used for the renewal of adult passports and for applicant requesting one year after issuance of a passport a name change, extension of validity for limited passports, or correction of a printing error.  Applicant requesting a change within one year after issuance of a passport will apply for a replacement passport using the form (DS-5504), while the form (DS-11) should be used for all first passport, minors (ages 16 and under) new and renewal and Lost/Stolen passport applicant.  Applicant requesting additional visa pages should use the form (DS-4085). 
In order to use the (DS-82) application form, you must meet all of the following criteria:
• You must have been issued a US passport in your name within the past 12 years; and
• You must submit your most recent US passport with your application; and
• You must have been age 16 or older when your most recent US passport was issued.
• A previous US passport is adequate evidence of both identity and citizenship. In lieu of a previous passport, identity can be proven with a photo I.D. issued by a competent authority. US citizenship, however, must be established by presenting either a certified copy of a US birth certificate, a Consular Report of Birth Abroad (if the applicant was born outside the US), or a US voter registration card. Federal law prohibits the issuance of a full-validity US passport to any person who lacks documentary evidence of US citizenship.
In addition to the completed passport application form, you will be required to submit:
• Two photographs measuring 2" x 2" (5cm x 5cm).
• Proof of your identity.
• Proof of your US citizenship.
• A passport application fee (see "Fees" below).
• Application that does not meet all of these criteria should be made using the DS-11 form, which requires the applicant to make a sworn statement before a consular officer.

How do I apply for a passport for my child?


• Parents or guardian applying for a minor’s new or renewal passport must in addition to the requirement for submission of (DS-82) shown above must submit:
• A completed form (DS-11) [Do not sign]
• Proof of U.S. Citizenship of the child which could be U.S. Passport etc
• Parental proof of relationship to the child for example certified foreign birth certificate, adoption decree, court order establishing custody etc.
• Provide Parental Identification which could be passport, drivers license etc.
• Both parents must appear together and sign the application or
• When only one parent appears, the parent should sign and submit a primary evidence of sole authority to apply along such as child’s certified U.S. or foreign birth certificate listing only applying parent or death certificate of non-applying parent, or Adoption decree etc
• If none of the above documentation is available, the applying parent/guardian should submit Form (DS-3053): A notarized Statement of Consent: Issuance of a Passport to a Minor Under Age 16.
• Each minor (child under age 16) child applying for a first time new, or renewal passport shall appear in person.
 
Passport Fees


Passport applications for children under age 16 cost US$ 85.00. A first-time application (Form DS-11) for an adult (age 16 and above) costs US$ 100.00, while an adult renewal application (DS-82) costs US$ 75.00. The fee for a replacement of a lost or stolen passport is US$ 100.00 for adults and US$ 85.00 for children under 16.  
All fees may be paid in US or Namibian dollars in cash (no checks or credit cards).
